Step 1: Initial Assessment
We begin with a comprehensive in-home or virtual assessment to learn about the client’s unique needs, health considerations, daily routines, and personal goals. This conversation allows us to develop the most appropriate and effective plan of care tailored specifically to the individual.
Step 2: Customized Plan of Care
Based on the assessment, we create a personalized care plan that outlines the type of support needed, recommended services, and the level of assistance required to promote comfort, independence, and well-being.
Step 3: Caregiver Matching
We thoughtfully match each client with one of our compassionate and supportive caregivers. We consider personality, experience, skillset, and compatibility to ensure a positive and trusting relationship from the start.
Step 4: Onsite Meet & Greet
Before services begin, we schedule an onsite Meet & Greet so the client and caregiver can get to know one another. This important step helps build comfort, confidence, and connection while allowing both parties to ask questions and discuss expectations.
Step 5: Service Schedule Confirmation
Once the client and caregiver establish a connection, we finalize the schedule of services based on the client’s preferences and care needs. Our team remains flexible and responsive, making adjustments whenever necessary to ensure continued satisfaction.
Ongoing Support & Communication
Our commitment doesn’t stop once care begins. We maintain regular communication with clients and families, monitor satisfaction, and adjust care plans as needs evolve to ensure consistent, high-quality service.
